A varicocelectomy is a minimally invasive surgical procedure performed to improve male fertility. It can normally be done on an outpatient basis using local or general anesthetic. A small incision is made in the groin close to where the testicles originally fell through the abdominal wall. The veins that produce the varicocele are identified and interrupted to remove the abnormal blood flow to the varicocele.

The goals of varicocele repair are to relieve pain in symptomatic cases and to improve semen parameters, testicular function, and pregnancy rates in couples with male factor infertility associated with varicocele.

Surgical varicocelectomy: This approach involves ligation (obstruction) of the testicular veins to stop the normal blood flow. The surgery is minimally invasive (laparoscopy) and requires a small incision just below the pubic hair line. An operating microscope is used to identify the abnormal veins as they course through the spermatic cord. Intra-operative ultrasound is used to spot and save the testicular artery. The surgery can typically be performed in less than one hour and recovery is quick with most men returning to work within one to two days.

Success rates of varicocelectomy can be measured in terms of resulting pregnancy ratesL: Seventy-two percent of men will do so after two years. This compares to 16% of men whose partners will conceive without undergoing a varicocelectomy during the same period.

Success rates can also be measured by change in semen analysis results. Generally, Sixty-five percent of men will show a significant improvement in the semen analysis within 12 months. A significant change is defined as a doubling of the total motile count. The total motile count is the calculated number of sperm that the man actually ejaculates.
